Passaic County Comprehensive Care Accountable Care Organization, Inc.

Welcome  The Passaic County Comprehensive Accountable Care Organization (PCC-ACO) is a partnership of St. Joseph’s University Medical Center, Eva’s Village, Straight and Narrow, Harbor House, Inc., and local area physicians to collaboratively work with healthcare providers, hospitals, and physician practices to improve the care and coordination of healthcare for Passaic County residents while decreasing costs.

To achieve this goal, PCC-ACO has submitted an application to the New Jersey Department of Human Services (NJ DHS), Division of Medical Assistance and Health Services to be certified as a Medicaid ACO. Below is a link to our application for public review and comment.

This pilot program will initially focus on residents living in Central Paterson (zip codes 07501,07502, 07503) with Medicaid or Medicaid HMO coverage. Over the course of the demonstration, PCC-ACO will seek to develop additional community partnerships as well as work with both NJ DHS and regional managed care organizations to expand service coverage to all eligible residents in the Greater Paterson Region.

What is an ACO?  Recent healthcare reform efforts, which have culminated in the Patient Protection and Affordable Care Act (PPACA), have increased the focus on improving care outcomes-primarily improving quality and reducing cost. A major component of PPACA requires that populations be compared across these outcomes.  

A fundamental component of population outcome management included in PPACA is Accountable Care Organizations (ACOs). Under this approach, clinicians will not only be compared on their performance with respect to populations under their care, but they also will be financially incentivized to deliver exceptional outcomes and perhaps, depending on the program, be financially penalized for inferior outcomes.  

The major way in which ACOs operate is that hospitals, physicians and other providers organize themselves to form an ACO and agree to accept responsibility for the care outcomes of a patient population.

ACOs are groups of doctors and other health care providers who voluntarily work together to give you high quality service and care at the right time in the right setting.
