Receiving a Birth Certificate for a Newborn

Please complete the birth certificate worksheet and bring it with you when you visit St. Joseph’s University Medical Center to deliver your baby. It is important to complete this form carefully – print clearly – because it will be used to create your baby’s legal birth certificate.

Once the form is reviewed and signed, we will send the document to the local office of vital statistics, where it will be registered with the state and permanently stored. You may receive a copy of your newborn’s birth certificate no earlier than 10 days after the birth of the child.

To receive the baby’s birth certificate, contact the Office of Vital Statistics in Paterson, New Jersey.
