St. Joseph’s Health is pleased to introduce a new tool to make requesting your medical history easy and efficient.  You can receive your medical information electronically within 24 hours thanks to Patient Request Tool – a user-friendly way to request the information you are seeking.

The St. Joseph’s Health Medical Records Department is open 8 a.m. - 4:30 p.m., Monday through Friday. To obtain a copy of a medical record, please print and complete our Authorization for Release of Information.

For all other SJH locations, including hospital outpatient clinics, mental health clinics, physician offices, radiology sites, and St. Joseph’s Healthcare and Rehab Center, please contact the individual site or St. Joseph’s University Medical Center.

St. Joseph’s Health will make every effort to prepare your medical records in a timely manner, but by law, we are allowed up to 30 days should any extenuating circumstances arise.
