Hospice/Palliative Care

Hospice care from a dedicated and caring team can make a meaningful difference for those facing end of life, and for their loved ones. The Hospice team can reduce physical pain, ease emotional and spiritual suffering, and facilitate financial and legal activities as needed.

At St. Joseph's Healthcare and Rehab Center, we contract with several agencies to provide hospice care to our residents. Once a decision is made for hospice services, the staff at St. Joseph's Healthcare and Rehab Center will arrange a meeting with the resident, their family, and the hospice agency to discuss hospice services to be provided at St. Joseph's.

As a team, St. Joseph's Healthcare and Rehab Center staff and the hospice staff develop an individualized plan of care, focusing on the comfort of the resident. Realizing that they are also going through a difficult time, the staff at St. Joseph's Healthcare and Rehab Center also provides comfort and assistance to family members.
