Here are the 7 easy steps to certification of CME activities. Please go through all of the CME application links. Be sure that any items indicated with “FORM” proceeding the title need to be filled out, saved and sent to cme@sjhmc.org. Do not send multiple emails. Please attach all completed documents to one email prior to submission.
Step 1: Contact the SJUMC Office of CME (part of Academic Affairs) prior to planning
The Activity Designee contacts SJUMC CME office to plan an educational activity for AMA PRA Category 1 Credit.
Step 2: Activity Designee submits a completed CME Application Form to cme@sjhmc.org
- Forms should be sent to cme@sjhmc.org no less than 1 month prior to the date of the activity (forms received less than 1 month prior to the date of the activity may not be approved or may incur an additional fee).
- SJUMC CME reviews application to ensure completion and provides feedback within ten business days.
Step 3: Please submit grant letter(s) of agreement for review and SJUMC CME signature as soon as possible.
Both the commercial supporter and SJUMC CME must sign the grant agreement prior to acknowledgement.
Step 4: Activity Designee prepares and submits to cme@sjhmc.org draft marketing activity materials (save-the-date card, brochure, email announcement, website, etc.).
- Please refer to Required Elements for Syllabi/Brochures to ensure that all CME information is included.
- SJUMC CME reviews and approves (5 business days following receipt).
Step 5: Activity Designee and SJUMC CME work together to ensure all CME disclosures have been completed by planning committee members/speakers/authors.
- SJUMC CME will review the disclosure information for each individual and take necessary steps to resolve any potential conflicts of interest prior to the activity using the following options:
- Speaker provides presentation materials to the SJUMC CME for review prior to presenting at the CME-certified activity.
- Note: if the activity is Internet-based or an enduring material, SJUMC CME will review and approve all content prior to the release of the activity.
Step 6: Activity Designee prepares and submits to SJUMC CME draft syllabus, enduring material, and evaluation.
- All CME disclosure forms must be received in order for materials to be approved.
- Please refer to Required Elements for Syllabus/Brochures or Enduring Materials to ensure that all information is included.
- SJUMC CME reviews and approves.
Step 7: Following the CME/CE activity, Activity Designee submits the following to cme@sjhmc.org within 14 days.
- Electronic copy of participant list who request CME credit
- Completed sign-in sheets for each day
- Evaluation summary (sorted into 2 categories: physicians and other healthcare professionals)
- 2 copies of the brochure, enduring materials, marketing materials
- Final budget report, listing all income and expenses
- Copies of commercial support grant checks, date paid, and/or check numbers
