At St. Joseph’s Health, coordinating patient care with community physicians and practices is important to us. To improve both the timeliness and quality of patient information shared outside of the hospital or clinic setting, SJH has selected Provider Directory and Message Delivery Services from careMESH.
With this program, SJH shares digital Admission and Discharge Summaries with the patient’s primary care provider (as designated in the patient’s chart).

- What is careMESH and what does it mean for me?
careMESH services were selected by St. Joseph’s Health so they could easily share information with you about your patients. This HIPAA-compliant solution was developed to improve how hospitals coordinate care with community physicians and practices outside of their facility or enterprise and make it easier for you to participate in patient care planning. The user interface was designed by doctors and allows you to receive a message and quickly gain access to take action.
- Is there a cost for me to receive messages?
St. Joseph’s Health invested in careMESH to improve how they can communicate with community providers, and to give you the opportunity to seamlessly reply. You do not need to pay for anything to participate in this program. - Is the system secure?
YES! careMESH was built with security as a top priority and has undergone a rigorous security approval process to be selected by the hospital to transmit patient health information. You can trust that careMESH abides by all regulations to protect access to, and the ongoing security of, data shared with the service. If you have questions, we invite you to review the Terms of Use and Privacy Policy contained within the system’s dropdown menu. - A hospital has sent me some patient information through careMESH. How do I login to the system for the first time?
Accessing careMESH is quick and easy, as long as you are the designated recipient of the patient information from the hospital. After receiving a direct secure message, email, or fax inviting you to login, simply follow the prompts to activate your account. Once there, you will see new messages in your “MAIL” inbox and you can read, download, and respond to content as appropriate. - Can I tailor careMESH to the needs of my practice?
YES! We understand that you are very busy and have well-designed processes that you wish to maintain. First, we encourage you to review the information in your individual careMESH directory listing to ensure completeness. Also, update your communications preferences (See the “PROFILE” and “SETTINGS” tabs) so that future messages from St. Joseph’s Health (or other careMESH subscribers) arrive the way you prefer.
- How do designate a member of my team to receive messages?
After you login, look for the “Delegates” and “Message Management” tabs to assign certain communications to members of your staff, copy some items to fax, and keep your practice running like a well-oiled machine! Your new delegate will receive an activation code to the email address you entered and can quickly get started. - What else can I do with my new careMESH account?
Once you’ve activated your account, we invite you to click around and learn more! In careMESH, you can receive notifications about your patient admissions to TGH, receive automated discharge summaries along with a patient record, view and upload records received from the hospital into your own EHR, and much more!
- Do I need a separate consent from the patient to share their health information through careMESH?
Use of the careMESH platform and services complies with the HIPAA Privacy Rule for protecting patient health information (PHI) in much the same way that you use secure email or fax today. The Privacy Rule permits the sharing of health information by healthcare providers, business associates of HIPAA-covered entities, and others covered by HIPAA Rules. In general, permitted uses and disclosures are for treatment, payment, or health care operations. - What are the benefits of careMESH to my practice?
Because St. Joseph’s Health chose to use careMESH to share patient information, we are providing you with a free, easy-to-use, digital account—so you or practice can:
- Receive and respond to digital communications about patients
- View patient records in a user-friendly format designed by doctors
- Designate a practice admin as a delegate to manage the information you receive to align with practice workflows
- Set your communications preferences to receive Admit/Discharge Summaries and other information according to your current workflows
