The St. Joseph’s Health Medical Records Department is open 9 a.m. – 4 p.m., Monday through Friday. To obtain a copy of a medical record, please use our Patient Central link above, or print and complete the Authorization for Release of Information.

St. Joseph’s Health will make every effort to prepare your medical records in a timely manner, but by law, we are allowed up to 30 days should any extenuating circumstances arise.
